Simply follow the instructions below to access your personal portal.
Once inside, you’ll easily be able to browse your files, download documents, review billing, schedule appointments, add yourself to a wait list and more.
Sign in with your e-mail and password credentials.
If you don’t have either, simply click FORGOT USER NAME or PASSWORD, or call the office at (817) 310-0393
Once inside, you’ll notice your options down the left side.
